What to look for

  • Get written instructions and know whom to call with medical questions.
  • Confirm medications with the discharge team and arrange a pharmacy pickup.
  • Know the date, location, and transportation plan for follow-up visits.
  • Check that walking routes, bathroom access, meals, and needed supplies are ready.
  • Decide who will help with dressing, bathing, food, and other daily tasks.

Taking the next step

Make a plan that fits your family.

Ask the discharge team to explain the recommended level of help at home, including any licensed services or equipment. Share this with family before the first day back.

Non-medical caregivers can assist with everyday routines and appointment transportation. A clinician should address symptoms, medication changes, wound care, and other medical needs.
